Postoperative management of severe acute anemia in a Jehovah's Witness.

摘要: Background Low levels of hemoglobin (Hb) are not rare in patients who refuse blood components but this case is unique due to the severity anemia and possibility that her previous episode acute normovolemic hemodilution has influenced outcome. Case Report We report an incident involving loss after surgery with extremely low hematocrit. Despite Hb (2.8 g/dL) she remained lucid, expressing wish receive transfusion. When patient lost consciousness (Hb, 1.4 g/dL) was promptly sedated, curarized, put onto mechanical controlled ventilation. Aggressive erythropoietin therapy increased patient's level by 240% 10 days, despite a high platelet count. This demonstrates critical oxygen delivery may be lower than previously thought possible. Conclusion This example resilience human body extreme circumstance. It might most severe person survived without any sequelae.
