Comparative Study of Collagenase and Papain-urea Based Preparations in the Management of Chronic Nonhealing Limb Ulcers

摘要: The objectives of this study are to compare effectiveness collagenase v/s papain- urea for debridement chronic non-healing ulcers/wounds and evaluate their role in promoting ulcer healing by granulation reduction ulcer/wound size. A comparative 100 patients was done at M.S. Ramaiah Hospitals India from November 2007 August 2009. Patients were selected, randomized, divided into two groups consisting 50 each. Group- 1 treated with Group 2 urea. evaluated 0, 1, 2, 3, 4 weeks size, granulation, discharge over all response treatment. mean age 42 +/- 15yrs. co-morbidities diabetes 28.4 %, hypertension 21.1%, others 14.0%. Culture sensitivity test reveals that most frequently grown organism E. coli (13%) Staphyloccus aureus (9%) samples no growth 64%. In papain-urea group, reduced 24.8 sq. cms 11.9 group 23.1 9.7 cms. There significant slough necrotic tissue i.e. 22.54 5.07 21.76 6.12 Significant amount increase i.e., 2.4 6.82 1.4 3.8 observed. But Papain-urea showed better 2nd, 3rd, 4th compared (p value <0.05) improvement 28% papain-urea, 12% group. Mean follow-up period 7.28- 8.14wks. have shown proven efficacy bringing out enzymatic wound debridement. is a debriding agent promotes faster collagenase.
