Early Results Using Sterilized Acellular Human Dermis (NeoForm) in Postmastectomy Tissue Expander Breast Reconstruction

摘要: Acellular dermal products play a beneficial role in immediate tissue expander breast reconstruction. They provide improved coverage and support the lower pole, allowing pectoralis muscle to drape over most of maximize pole expansion. Tissue incorporation is desired without any adverse affects on recovery. The purpose this series evaluate early safety morbidity preserved human allograft dermis sterilized using Tutoplast(R) process.All patients who underwent reconstruction with NeoForm(R) at Emory University Hospital between 6/07 4/08 were included series. Patient demographics, risk factors, surgical technique, complications outcomes evaluated.Twenty-two consecutive included, total 31 breasts (bilateral n=9, left right n=4). average age was 48 years (range: 31-71), an body mass index 26.7 19-35). Fifteen had diagnosis invasive cancer 7 DCIS. Risk factors hypertension n=5, history smoking n=2, diabetes n=1, post operative radiation therapy n=8. All reconstructions expanders. NeoForm rehydrated for appropriately 3-5 minutes. It sutured superiorly border inferiorly inframammary fold. 4x16 size used 18 breasts, 6x16 cm 13 breasts. Early occurred one patient native mastectomy skin necrosis. drains removed by third post-operative week. There no cases infection, foreign reaction, rejection, seromas, or erythema. expansion performed difficult. Expander removal secondary implant insertion demonstrated adequate 16 patients. Encapsulation Neoform, infection extrusion not observed.Acellular have become useful addition successfully reconstructions. Good observed clinically there that could be related NeoForm.
