Medical and ethical considerations in uterus transplantation

摘要: Transplanting a uterus has unique characteristics, since successful outcome is represented only by the birth of viable healthy child. For this reason, critical issues in type transplantation differ profoundly from those other solid organs and, beside functioning uterus, involve 3 additional steps. First, at time implantation, quality embryo tested specialized decidual cells surrounding implanting embryo; such testing aimed allowing development normal embryo. Second, early gestation onward, blood supply to increases 45 750mL per minute. Vascular anastomoses should support marked increase flow. Third, full transformation spiral arterioles placental bed required direct 75% uterine flow intervillous space. Unfortunately, no suitable animal model available for experimentation. Three overarching ethical must be considered. Should organ transplant conducted when it not absolutely necessary as life-saving or quality-of-life-saving measure? To what extent medicine delimit its potential spite societal desires? society demand application whichever technology can developed if so, extent?
