CLINICAL DIAGNOSIS AND ASSESSMENT OF SEVERITY OF CONFIRMED DENGUE INFECTIONS IN VIETNAMESE CHILDREN: IS THE WORLD HEALTH ORGANIZATION CLASSIFICATION SYSTEM HELPFUL?

摘要: Classification of dengue using the current World Health Organization (WHO) system is not straightforward. In a large prospective study pediatric dengue, no clinical or basic laboratory parameters clearly differentiated between children with and without although petechiae hepatomegaly were independently associated diagnosis. Among 712 dengue-infected there was considerable overlap in major features. Mucosal bleeding observed equal frequency those fever hemorrhagic (DHF), petechiae, thrombocytopenia, tourniquet test poorly two diagnostic categories. Fifty-seven (18%) 310 shock did fulfill all four criteria considered necessary for diagnosis DHF by WHO, but use WHO provisional classification scheme resulted over-inflation figures. If separate entities truly exist rather than continuous spectrum disease, it essential that some measure capillary leak included any system, less emphasis on specific platelet count.
