Epidemiological and surgical aspects of urogenital fistulae: A review of 25 years' experience in southeast Nigeria

摘要: The aim of the study was to determine epidemiological background, clinical details and surgical outcome patients presenting with urogenital fistulae St Luke's Hospital, Uyo, associated VVF Unit at Mbribit Itam, Akwa Ibom State, Nigeria, between January 1970 December 1994. A retrospective review hospital operating theater records case notes carried out. Clinical were assessed for total cohort 2484 patients. Epidemiological data extracted from 715 1990 Of these 92.2% obstetric etiology, 80.3% following neglected obstructed labor, 6.9% cesarean section, 5.0% followed ruptured uterus; 4.4% pelvic surgery remaining 3.4% miscellaneous causes included malignancy, coital injury, infection trauma; 8% had a coexisting rectovaginal fistula or third-degree perineal tear. Only 37.3% aware their age; median age this group 28 years. Literacy difficult judge reliably, although 29% able sign name. Parity ranged 0 17, only 31.4% related first pregnancies. Although 73.1% delivered in hospital, 97.1% labor initially managed home, traditional birth attendant, maternity church; 34.1% by live-birth rate 10.3% causative pregnancy. For variety reasons 124 women not operated upon: 1954 underwent one operation, giving presumptive cure operation 81.2%; 247 two, 116 three, 32 four, 11 five operations during period. ultimate closure 97.7%, 0.6% undergoing urinary diversion. type distribution recorded series is consistent previous largely developing world. Surgical rates are also comparable. background variance reports several respects; may reflect biosocial differences population studied.
