Chronic motor cortex stimulation for central deafferentation pain: experience with bulbar pain secondary to Wallenberg syndrome.

摘要: Six patients with bulbar pain caused by lateral medullary infarct were treated stimulation therapy. Dysesthesia on the opposite side of body was subjected to therapy in these patients. Of subjects, 4 underwent thalamic and 3 motor cortex stimulation; effects both investigated 1 patient. No satisfactory control obtained any this series. In contrast, 2 reported control. The inhibition usually occurred at intensities below threshold for production muscle contraction (pulse duration, 0.1–0.5 ms; intensity, 3–8 V). This finding consistent our previous observations made a series pain, indicating that is significantly more useful than controlling deafferentation secondary central nervous system lesions. We discuss possibility better may be provided level rostral site deafferentation.
