Mechanical Circulatory Support in Management of Cardiogenic Shock and Myxedema Coma

摘要: The cardiovascular system is a major target of thyroid hormone action and the two systems are closely interlinked. It can be greatly impacted even with subtle alterations in function. Caution needed when implementing replacement patients severe hypothyroidism, especially setting ischemic coronary artery disease. If not properly treated, myxedema may ensue. Given high mortality coma, supportive care always suffice require more invasive interventions. We present challenging case patient overt hypothyroidism concurrent acute syndrome which subsequently lead to coma cardiogenic shock. A transcaval approach for delivery an Impella 5.0 (Abiomed Inc., Danvers, MA) was utilized supporting this patient. To our knowledge, first reported that describes use mechanical circulatory support treating myxedema-induced collapse.
