摘要: This article describes a splint management program for zone 4-7 extensor tendon repairs that allows immediate controlled active motion (ICAM) of the repair and greater arcs adjacent digits. The is designed to relieve tension on tenorrhaphy by positioning involved digit in slight metacarpophalangeal joint hyperextension relative uninvolved digits with simple yoke control joints second wrist position. Cadaver intraoperative trials support this technique, 140 patient cases managed over 20 years. majority patients achieved rating excellent both digital extension flexion as judged Miller's criteria. There were very few lags no ruptures. Patients returned work ICAM average 18 days. time complete was seven weeks after repair, required an eight therapy visits. results study demonstrate splinting technique safe, manage, decreases morbidity associated immobilization, cost effective, has high compliance when compared other early programs.
