Clinical practice of medication reviews in institutional care settings for older people in the Netherlands: an explorative survey

摘要: Introduction Patient-specific medication reviews might be useful for correcting prescribing omissions. The Dutch Healthcare Inspectorate (IGZ) demands that a review is performed by pharmacist in cooperation with physician all residents of nursing homes (twice year) and residential (once year). Objective This study aims to show the clinical practice older people pharmacists. We asked following research questions. (i) To what extent are as required IGZ? (ii) How much time needed perform reviews? (iii) In opinion pharmacist, economically efficient medically relevant? Methods A web-based survey was sent 87 hospital pharmacists 270 community Results ‘best case vs worst case’ scenario, 76% 42% IGZ-required performed. Considering mean 730.7 per year spent on one (29.3 min), would need spend 2.5 months meet IGZ requirements. Almost every considers relevant, but only 47% consider them efficient. Conclusions shows institutional care settings do not requirements, which surprising considering review. Automation processes should initiated increase efficiency.
