Limited sampling strategies for estimating cyclosporin area under the concentration-time curve: review of current algorithms.

摘要: Cyclosporin, the drug of first choice in transplantation surgery, is characterized by a low therapeutic index and variable absorption, so close monitoring required to optimize dosing. Predose blood cyclosporin levels are measured routinely for monitoring, but this approach not optimal because area under concentration-time curve (AUC) correlates better with clinical events. However, conventional methods measuring AUC require many samples, which viable routine setting. can be simplified use setting using limited sampling strategy (LSS) that allows estimated small number samples collected at specific times. This article reviews current literature on estimating LSS. Thirty-eight papers suggesting time points were found. LSS has been developed different transplant types, dosing regimens, assays. Most authors suggested either two- or three-sample equations. Results from who validated their models suggest equations defined one type may applicable other both adults children, early late after transplantation. Moreover, it seems there flexibility available clinicians. The collect accurate estimations matter debate, wise minimize number. validation discussed.
