Old habits die hard: a case study on how new ways of teaching colonoscopy affect the habitus of experienced clinicians

摘要: Objectives The purpose of this study is to explore the habitual constraints and opportunities that affect how experienced clinicians learn new skills and, in particular, ways teaching can influence these. Methods We conducted a case based on specialized training program for colonoscopy services Denmark. Data was obtained from short-term ethnographic fieldwork in-depth interviews during program. Participants were 12 colonoscopists three expert trainers Denmark UK. analysis data involved categorization, inductive coding, theoretical reading inspired by sociological theory. Results clinicians' responsiveness shaped an underlying logic practice characterized tacit skills, routine work, lower status, skepticism self-protectiveness. In order overcome these constraints, applied pedagogical approach four methods: 1) intellectualization: 'academization' competencies, 2) sensing scaffolding: hands-on experiences learning doing, 3) asymmetry: accentuating authority respect trainer, 4) relation-building: building relationship engagement between trainer clinician. This multi-dimensional enabled create buy-in (so-called illusio). Conclusions Clinical include socially constructed behaviors unconscious competences which continuing medical education. suggests educational strategies may help establish logics improve conscious competence.
