Aggressive arterial reconstruction for critical lower limb ischaemia.

摘要: A consecutive series of 315 patients underwent arterial reconstruction for 329 critically ischaemic lower limbs over a 5-year period. Patients were not excluded from limb salvage surgery on the basis poor run-off preoperative angiography. Femorocrural bypass to single calf vessel was required in 239 (73 per cent); 30-day cumulative mortality rate 7 cent, rising 41 cent at 5 years. Cumulative graft patency 30 days, 1 year, 2 years and 96, 85, 84 82 respectively independent level reconstruction. In situ long saphenous vein conduit choice distal but, when this absent, satisfactory results obtained with arm or composite grafts. Acceptable can be reconstructive without angiographic selection patients. An aggressive approach is therefore justified.
