摘要: Objective This paper explores the lessons learned from a series of three randomized controlled trials that included 498 community-living frail older adults (≥65 years) using home care services in Southern Ontario, Canada. Each study was designed to evaluate effectiveness different multi-component nurse-led health promotion and disease prevention (HPDP) interventions. Methods The HPDP interventions were 6- or 12-month evidence-based strategies targeting known risk factors for functional decline frailty. Across studies, common approach used measure change health-related quality life (HRQOL) (SF-36) costs use (Health Social Services Utilization Inventory) baseline end intervention. Results main lesson studies is clients provide greater improvements HRQOL compared with usual care. Such approaches are highly acceptable this population can be implemented existing resources. Nurse-led should include multiple visits, multidimensional screening assessment, strategies, intensive case management, inter-professional collaboration, providers geriatric training experience, referral coordination community services, theory use. Conclusion results underscore need reinvest optimize promote ageing place target adults. More needed additional other contexts settings.
