Partial Rotator Cuff Tears in Adolescents: Factors Affecting Outcomes

摘要: Introduction: In the adult population, rotator cuff tears are common and established treatment methods yield satisfactory results. adolescents, however, these injuries uncommon few outcome reports exist. The purpose of this study was to examine a series adolescent tears, identify associated pathology, report outcomes. Methods: A retrospective comparative analysis patients treated for diagnosed by magnetic resonance imaging (MRI) or arthroscopy between 2008 2010 performed. Patients were divided rendered: nonoperative operative. Demographic diagnostic variables compared 2 groups. After release full activity, 3 patient measures obtained: QuickDASH (Disability Arm, Shoulder, Hand), Sports module, Single Assessment Numerical Evaluation (SANE). Results: Fifty-three adolescents (38 boys 15 girls) with mean age 15.8 years (8.8 18.8 y) met inclusion criteria. All partial articular-sided tendon avulsions, surgical (when required) consisted debridement stable edges. underwent trial at least 6 weeks physical therapy, 57% failing improve requiring subsequent surgery. that nonoperatively, 39% pathology based on MRI findings, whereas operative exhibited an rate 70%. MRI-diagnosed 1.8 times more likely (95% confidence interval, 1.02-3.13, P=0.025) require surgery those without MRI-identified pathology. Nineteen (13 operative, nonoperative) completed questionnaires 16.9 months after treatment. QuickDASH, SANE, module scores not statistically different groups (7.5 vs. 8.1, P=0.90; 85.3 80.6, P=0.47; 5.2 19.5, P=0.39, respectively). significant correlations one another, strongest correlation being negative association SANE (r=−0.76, P=0.001). Discussion: Isolated avulsion may be successfully return sports expected; if present then less successful. Improvement in pain activities daily living can achieved failed conservative management injuries; athlete will often have residual shoulder complaints during participation.
