摘要: Background Integration of care for children with complex needs is developing slowly internationally. There remains wide variation in the governance of, and access to, these their families. Local problem was a need to develop service that would have specific remit organising overall management community. Method A bespoke model established specifically support healthcare (CHNs). The sole focus team provide highest standard families, enable families remain central decision-making. Intervention CHNs August 2017 appointment manager case managers. comprehensive training education programme put place Results viewed as delivering high-quality care. Parents stakeholders highlighted value placed within on individualised care, specialist knowledge importance advocacy. Conclusions recognises exceptional lives live, given complexities challenges they overcome daily basis. built skill set supporting others involved child, are solely employed dedicated provision CHNs. corporate structures seem strong stand up scrutiny very well terms parents’ stakeholders’ perspectives context published international best practice.
