Role of disability-case manager for chronic diseases: Using the ICF as a practical background

摘要: Purpose. To report on the case manager's activity in a hospital setting as supporting professional for families that need to deal with different services and professionals get answers their health psychosocial needs.Method. A qualitative analysis interpretation based manager observations ICF checklist evaluation medical rehabilitation were employed.Results. The study presented aimed show one of most typical interventions manager: creation network around person complex multifaceted needs, where this does not exist.
