まれな小腸原発悪性神経鞘腫(malignant schwannoma)を含む異時性四重複悪性腫瘍の1例

摘要: 今回, 我々は非常にまれな小腸原発悪性神経鞘腫(malignant schwannoma)を含む, 異時性四重複悪性腫瘍の1例を経験したので報告する. 症例は79歳の女性. 既往歴としてS状結腸癌, 甲状腺癌があった. 強い腹痛と腹部膨満を主訴に受診, 汎発性腹膜炎の診断で緊急開腹術となった. 開腹時, 小腸に穿孔性の腫瘍を認めたため小腸切除術を施行した. 腫瘍は病理組織学的に非上皮性で, 大小不同, 多形性, 不整分裂像をもった細胞より成り立ち, 免疫染色上, S-100, NSE, vimentinが陽性, MSA, desmin, CD-34, c-kitが陰性であった. 以上より小腸原発悪性神経鞘腫と診断した. 術後経過中, 食道癌(第4癌)も発見されたが, 全身状態悪化のため死亡した. 本症例は異時性にS状結腸癌, 甲状腺癌, 小腸悪性神経鞘腫, 食道癌(同時性)の四悪性腫瘍を重複した非常にまれな症例と考えられた. 本症例における神経鞘腫の診断には免疫染色が有用であった.
