Blood pressure response to melatonin in type 1 diabetes

摘要: :  Hypothesis:  Diminished decline of blood pressure at night has been associated with increased risk cardiovascular disease. Exogenous melatonin given to healthy normotensive adults reduces pressure. We hypothesized that increases the during sleep in adolescents type 1 diabetes. Patients and methods:  Eleven normoalbuminuric patients diabetes 7.0 ± 3.5 yr duration 10 controls aged 14–18 yr participated a randomized placebo-controlled double-blind crossover study 5 mg for 1 wk, 1-wk washout. Blood was measured every 20 min 24 h by an ambulatory device on day before last each treatment. Sleep measures were recorded diary wrist activity meter. grouped 24 h, sleep, wake periods. Statistical analysis used duration, number awakenings, treatment sequence as covariates comparisons treatment. Results:  In diabetes, diastolic significantly greater (17.8 ± 4.4 mmHg) than placebo (16.0 ± 3.0 mmHg, p < 0.01). No significant drug effect present controls. side effects noted. Conclusion:  Melatonin amplifies nocturnal diabetes. propose dose–response studies short- long-acting preparations be conducted considered trials prevention hypertension
