摘要: Summary Objective A partnered medication review and charting model involving a pharmacist medical officer was implemented in the Emergency Short Stay Unit General Medicine of major tertiary hospital. The aim study to describe safety effectiveness this setting. Methods developed. Credentialed pharmacists charted pre-admission medications venous thromboembolism prophylaxis collaboration with admitting officer. subsequently had clinical discussion treating nurse regarding management plan for patient. prospective audit undertaken all patients from initiation service. Results total 549 by 14th November 2012 30th April 2013. 4765 were 7 identified errors, corresponding an error rate 1.47 per 1000 charted. Conclusions Partnered unit is achievable, safe effective. Benefits extend beyond medications, value added admission process through early Further research required provide evidence further support collaborative model.
