摘要: OBJECTIVE--To review the efficacy of routine prenatal ultrasonography for detecting fetal structural abnormalities. DESIGN--Retrospective study ultrasonographic findings and outcome all pregnancies in women scanned 1988-9. SETTING--Maternity department a district general hospital. SUBJECTS--8785 fetuses. MAIN OUTCOME MEASURES--Correlation with neonate. RESULTS--8733 babies were born during 1988-9, 52 terminated after malformation was identified. 8432 (95%) fetuses examined by second trimester. 130 (1.5%) found to have an abnormality at birth or termination pregnancy, 125 which had been In 93 cases detected before 24 weeks (sensitivity 74.4%, 95% confidence interval 66.7% 82.1%. Two false positive diagnoses occurred, both not apparently normal infants born. This gives specificity 99.98% (99.9% 99.99%). The predictive value trimester 97.9% (92.6% 99.7%). Of abnormalities, 87 lethal severely disabling; 72 screening programme 82.8%, 73.2% 90.0%). CONCLUSION--Routine examination low risk population detects many abnormalities but can present several dilemmas counselling.
