Glaucoma care and clinical profile in Priest Hospital, Thailand.

摘要: Objective: T o assess the prevalence, mechanism and status of glaucoma, to investigate magnitude visual impairment from glaucoma its relating factors in Buddhist priest novices. Material Method: Cross-sectional study 190 patients treated Glaucoma service, Priest hospital was performed. One hundred thirty seven with suspected had comprehensive ophthalmic examination included interview on medical ocular history, acuity, applanation tonometry, gonioscopy, optic disc, field evaluation, retinal nerve fiber layer thickness measurement (Stratus Optical Coherence Tomography; Stratus OCT). Results: diagnosed 106 (77%) (181 eyes); 31 (23%) were suspects. Open angle (OAG) found 53 (50%) cases classified into 36 (33%) primary open 17 (16%) normal tension (NTG). Five (4.6%) pseudoexfoliative 24 angle-closure (PACG), 10 (9.4%) angleclosure secondary other causes (SACG), (6.6%) juvenile glaucoma. Among suspects, 18 based disc appearance, eight intraocular pressure (IOP), two three closure (PAC). The prevalence increased age, highest age range 71 80 years. service 53% priest. Rate higher rural area than those Bangkok urban area. mean baseline IOP 26.5 ± 14.7 mmHg. 14.5 7.9 average deviation (MD) -14.45 11.11. OCT showed RNFL 70.8 35.6 µm. medications received by 72% priest, 18% laser treatment, 22 % surgery. At diagnosis, blind according WHO criteria, 29 (28%) unilaterally blind, (6.5%) low vision. After (29%) unilateral blindness none bilateral blindness. main associated diseases hypertension, dyslipidemia, diabetes. Transportation financial condition major barriers receiving eye care. Conclusions: OAG comprised 50% all 23% PACG, 16% subtypes rose significantly age. In patients, 47% retained good outcome. Delayed diagnosis a factor for unsatisfactory outcome as 28% monocular at previously unaware their disease.
