摘要: Metastatic disease to regional lymph nodes (LNs) is common in papillary thyroid carcinoma (PTC). LN dissection increasingly performed as part of the surgical management PTC. The role prophylactic central neck (pCND) PTC unclear. There limited evidence support a routine pCND clinical setting for nodal negative (cN0) aim this review was examine pros and cons cN0 In summary, advantages are: removal LNs that potentially harbor micro-metastases, more accurate staging order plan individualized management, reducing need re-operation remove metastatic which have developed later possible improvement overall survival. disadvantages an extensive surgery but lack survival benefit, higher incidence complications with little impact on local recurrence rate, possibility over treating patients it does not sound like cost effective approach small cancer. Considering low frequency permanent morbidity, some authors believe safe experienced hands even though its prognostic benefit has yet be demonstrated.
