摘要: Pharmaceutical care started in the nineties United States and has rapidly extended many other countries. Although there are different trends, such as clinical pharmacy services, cognitive medication management, review, they all share same philosophy objectives, namely "the responsible provision of drug therapy for purpose achieving definite outcomes that improve a patient's quality life". To attain these pharmaceutical process to be followed point-by-point order detect possible medication-related problems. Furthermore, pharmacists have work together with patients, ultimately physicians establish plan. This methodology requires basic skills documentation communication therefore, it is important implementation programs aimed at community-, hospital-, consultant pharmacists, consider PC element University teaching postgraduate studies.
