摘要: Malnutrition in hospitals often goes unrecognized. At present, no nutrition screening tool provides satisfactory re-sults identifying nutritional risk. Most tools depend on weight and height as criteria for diagnosing malnutrition. Weight is not recorded many patients some are time-consuming. An inclusive form (Nutrition Alert Form, NAF) was developed validated. NAF modified from the original version of Subjective Global Assessment (SGA) by adding two standard laboratory tests. The severity symptom changes were scored. validated 210 hospitalized Thai at Ramathibodi hospital an experienced clinical nutritionist (physician) hospital. Cross validation carried out between dietitian nurse another 90 patients. time nurses could complete a patient within 5 minutes. One four be weighed admission. scores 11 selected cut-off different malnutrition levels due to their high sensitivity, specificity accuracy 6 10 defined moderate diagnostic agreement "normal mild malnutrition", "moderate "severe malnutrition" 85%, 70% 72%, respectively. easy use, concise, does require expertise can used whether or body taken.
