Strategies to reduce deep sternal wound infection after bilateral internal mammary artery grafting

摘要: Deep sternal wound infections (DSWI) continue to be an infrequent but potentially devastating complication after cardiac surgical procedures. Its prevalence is more coronary artery bypass grafting using single internal mammary (IMA) graft. Bilateral (BIMA) harvesting carries a higher risk of infection than IMA. Several factors have been identified with and few are modifiable. Strategies that reduce DSWI target the modifiable include microbiological factors, appropriate antibiotic prophylaxis, tight glycemic control. Surgical strategies following BIMA harvest techniques IMA lesser devascularization sternum skeletonized, semiskeletonized modified pedicle associated greater preservation blood supply closure stability techniques. The various minimize during preoperative, intra postoperative periods summarized in this article.
