摘要: Introduction: Raoultella ornithinolytica is a Gram‐negative bacillus. This bacillus was until recently often confused with Klebsiella spp. It known primarily for causing histamine fish poisoning or scombroid syndrome, and rarely causes human infections. case report discusses patient who presented generalized primary peritonitis due to R. ornithinolytica. Case presentation: A 53‐year‐old Scandinavian man resident in Botswana, sub‐Saharan Africa, the emergency department shortly after onset of lower abdominal pain dysuria. He treated urinary tract infection discharged. came back 16 h more severe, fever 38 °C. Abdominal sonography unremarkable. Examination confirmed peritonitis, particularly marked right iliac fossa. had mild leukocytosis 13.1×109 l−1, C‐reactive protein level 372.7 mg l−1 his serum albumin 31 g l−1. admitted parenteral antibiotics, laparoscopic peritoneal lavage appendectomy. 500 ml free pus abdomen no evident focal source. swab grew The pathology showed early acute appendicitis severe periappendicitis suppurative peritonitis. discharged home 7 days antibiotics. Conclusion: infections are rare humans, but they can be life‐threatening.
