Clinical evaluation diagnostic intrasynovial anaesthesia isolated to hind metatarsophalangeal region. Radiography ultrasonography revealed subchondral cystic lesion apical portion lateral proximal sesamoid bone associated with oedema synovial effusion joint. Arthroscopic surgical debridement resulted in immediate post operative improvement score. Rest controlled exercise achieved complete resolution being able start training without apparent lameness. Subchondral lesions bones can be cause Surgical favourable outcome this case.
