摘要: In the first part of following chapter, important principles to consider when caring for patients with multiple conditions will be presented and applied a patient case. These include eliciting preferences, incorporating evidence, estimating prognosis, considering treatment complexity feasibility, optimizing prioritizing care plan. How these help foster patient-centered approach explored. second models that support some tools shared decision-making polypharmacy management in this population reviewed.
