摘要: The purpose of this article is to describe Joint Commission's 7 foundations safe and effective transitions care home: (a) leadership support; (b) multidisciplinary collaboration; (c) early identification patients/clients at risk; (d) transitional planning; (e) medication management; (f) patient family action/engagement; (g) transfer information.
