Raised tryptase without anaphylaxis or mastocytosis: heterophilic antibody interference in the serum tryptase assay

摘要: Mast cell tryptase (MCT) is a key diagnostic test for mastocytosis and anaphylaxis. High serum levels are also one of the risk factors adverse reaction in venom immunotherapy, yet occasional patients seen with raised absence either diagnosis. False positive results can be due to assay interference by heterophilic antibodies such as rheumatoid factor (RF) human anti-mouse (HAMA). We therefore investigated antibody activity HAMA cause MCT Phadia assay. Serum samples from 83 were assayed before after use blocking tubes (HBT). Samples more than 17% reduction detectable RF then HAMA. Fourteen (17%) showed >17% decrease mast HBT blocking. Post-HBT, eight 14 (57%) reverted elevated normal range values falls up 98%. decreased significantly (up 75%). Only tested was apparently affected IgM RF. In conclusion, any suspicious result should checked evaluate possible interference. caused factor. suggest strategy identifying interference, show that it essential incorporate this caveat into guidance interpretation results.
