摘要: The purpose of preventive home visits is to promote overall health and wellbeing in old age. aim this paper was describe the process development evidence-based visits, targeting independent community-living older persons. evidence base generated from published studies practical experiences. results demonstrate that should be directed persons 80 years involve various professional competences. personalized, lead concrete interventions, followed up. areas assessed derive a broad perspective include social, psychological, medical aspects. Core components protocol developed study captured physical, medical, psychosocial, environmental Results pilot showed validly identified risks among people with different levels ADL dependence.
