Cross Disciplinary Role Agreement is Needed When Coordinating Long-Term Opioid Prescribing for Cancer: a Qualitative Study.

摘要: BACKGROUND Cancer pain is highly prevalent and often managed in primary care or by oncology providers combination with providers. OBJECTIVES To understand interdisciplinary provider experiences coordinating opioid management for patients chronic cancer-related a large integrated healthcare system. DESIGN Qualitative research. PARTICIPANTS We conducted 20 semi-structured interviews two academically affiliated VA Medical Centers their associated community-based outpatient clinics. Participants included (PCPs) oncology-based personnel (OBPs). APPROACH deductively identified 94 examples of coordination cancer the interviews. secondarily used an inductive open coding approach themes through constant comparison coming to research team consensus. RESULTS Theme 1: PCPs OBPs generally believed one should handle all prescribing specific patient, but did not always agree on who that prescriber be context pain. 2: There are special circumstances where having multiple prescribers appropriate (e.g., crisis). 3: A collaborative process would include real-time communication negotiation between around will prescribing. 4: Providers barriers across disciplines. CONCLUSIONS Our findings highlight how about roles needed clinicians. Lack cross-disciplinary role agreement may result delays clinically management.
