摘要: The subclassification of schizophrenic disorders according to four diagnostic systems (DSM-III-R, ICD-10, the positive vs. negative dichtomy and Schneider's first rank symptoms) was compared over long-term course disease in 148 narrowly defined patients. A total 595 episodes were classified a mean observation period 23 years (range 10–50 years). Initially, paranoid/positive subtypes predominated, while later fulfilling symptomatological criteria residual/negative became more frequent. Disorganised/hebephrenic catatonic found be rare. Some premorbid features investigated as non-symptomatological validators for subclassification. Significant differences with regard age at onset. Patients whose episode paranoid or had highest initial disorganised/hebephrenic “residual” most unfavourable social adjustment, even when influence onset discounted. showed similarities result underlying concepts. Methodological implications are discussed.
