Traitement adjuvant du cancer colorectal : l’aspirine, une biothérapie ciblée ?

摘要: Malgre l’efficacite des chimiotherapies adjuvantes dans le cancer colorectal (CCR), la moitie patients ayant recu une chirurgie a visee curative vont presenter recidive. Il parait donc essentiel de trouver therapies plus efficaces. La prise reguliere d’aspirine diminue l’incidence CCR et adenomes. Cet effet antineoplasique passerait au moins en partie par modulation voie PIK3/AKT via l’inhibition cyclo-oxygenase 2. Recemment, deux etudes retrospectives suggerent fortement que l’aspirine consommee faible dose apres diagnostic d’un aurait un protecteur majeur sur risque recidive chez les presentant tumeur avec mutation PIK3CA, cependant ces notions n’ont pas ete confirmees etude recente presentee l’ASCO GI 2014. Sa toxicite, specialement sujet âge, pourrait limiter son utilisation pratique clinique. Une evaluation prospective randomisee exerese PIK3CA est requise.
