New Delhi Metallo-beta lactamase (NDM-1) in Enterobacteriaceae: treatment options with carbapenems compromised.

摘要: Background: Carbapenems are among the few useful antibiotics against multidrug resistant gram negative bacteria particularly those with extended spectrum beta lactamase. However resistance to carbapenems occurs and is mediated by mechanisms like loss of outer membrane proteins production lactamase that capable hydrolyzing carbapenems. An alert issued in UK 2009 warned an increasing number carbapenem Enterobacteriaceae strains identified hospital patients. Many them were recently hospitalized India Pakistan had new type metallo designated as New Delhi Metallo-1 (NDM-1). Objective: To assess NDM-1 Metallo enzyme at a tertiary care centre Mumbai. Materials Methods: Consecutive isolates collected from August November 2009. Susceptibility testing for was performed disc diffusion method. Carbapenemase confirmed Modified Hodge test. These then subjected single target PCR. A 475bp product amplified NDM primers visualized on 3% agarose gel. Results Conclusions: test positive all isolates. Of 24 22 producers while 2 non producers. Amongst producing organisms 10 Klebsiella spp, 9 Escherichia coli, Enterobacter spp 1was Morganella morganii. This high relatively short span worrisome trend compromises treatment options
