Multidisciplinary medication review: evaluation of a pharmaceutical care model for nursing homes

摘要: Objective The objective of this study was to assess implementation a pharmaceutical care model for the multidisciplinary elderly patients in nursing homes. Setting Prospective study, medication review, from January 2007 December 2009 ten homes affiliated Pharmacie interjurassienne (PIJ), Switzerland. Method Medication use data were collected and reviewed by pharmacist, focusing on drug indication, dosing, side effects, renal/hepatic elimination interactions. Drug-related problems (DRPs) discussed face-to-face with responsible physician nurse. issues formulated interventions proposed during meeting. DRPs documented using Pharmaceutical Care Network Europe scheme version 5.00 (PCNE V5.00). economic impact service estimated through retrospective evaluation annual costs. A satisfaction conducted among practitioners nurses. Main outcome measures DRPs, interventions, treatment changes implemented. Results Drug therapy 329 reviewed. number medicines per patient ranged 2 27 (mean 12.8). total 1,225 detected review led 343 medical evaluations secondary drug-drug interactions 803 adaptations: 373 drugs stopped, 197 dosages changed, 95 instructions amended, 86 choices altered, 35 formulations changed 17 new started. According Anatomical Classification System, main classes involved related Alimentary tract metabolism (n = 285), Nervous system 189) Cardiovascular 115). Since outset PIJ, costs decreased including whereas it stable other showed very positive appreciation Conclusion an efficient model, well accepted physicians It also indicated that patients, continuous contributed improved therapy, reduced unnecessary polypharmacy
