Intraoperative Periprosthetic Fractures in Total Hip Arthroplasty in Patients With Sickle Cell Disease at King Fahad Hospital Hofuf: A Cross-Sectional Study

摘要: Background  Patients with avascular necrosis related to sickle cell disease (SCD) can be severely disabled by the severe degenerative changes of their hip. Total hip arthroplasty (THA) remains the only surgical option for some these patients. a challenging procedure, and SCD patients demonstrate high percentages medical, intraoperative, postoperative complications implant failure. Furthermore, need THA following in Eastern Province Saudi Arabia is high, subsequent risk periprosthetic fracture prevalent. Therefore, it crucial conduct such study. Aim study This cross-sectional retrospective aimed assess prevalence associated factors fractures during total at King Fahad Hospital Hofuf, Arabia. Methods We collected data all who had undergone period, January 2015 September 2020. Forty-nine period were included. hip hemiarthroplasty, fractures, or had an indication other than excluded. Surgeon factors, assistant technique also then analyzed according gender, age, BMI, American Society Anesthesiologists classification, fixation type, stage, proximal femoral morphology, Vancouver classification preoperative hemoglobin (Hb) level, fractures. Descriptive statistics presented using frequency categorical variables, continuous variables summarized means ± standard deviations. Independent t-tests chi-square tests used test associations between variables. At 0.05, significance level was set. Results  Of patients, 32.7% were male 67.3% female. 32.7% advanced due necrosis. Among 20.4% intraoperative fracture, 90% class A, 10% B1. According Dorr 75.5% classified as A 24.5% B. 48 uncemented implant, 1 cemented. The mean perioperative Hb 9.02 + 2.02, minimum 6 maximum 14. No significant found incidence demographic operative profile characteristics. However, significantly higher rate observed operated on right side compared left side. Conclusion fracture among Hofuf period. Even adequate management, orthopedic surgeons must prepared deal rates fracture. association fracture in profiles.
