摘要: Objective: To assess the efficacy of an antibiotic protocol to avoid empirical use third-generation cephalosporins in community-acquired pneumonia (CAP). Design and setting: Retrospective case review patients with CAP one year after implementing protocol. Comparison was made treated at a metropolitan tertiary referral hospital (where common). Participants: 86 (district protocol) 72 (metropolitan hospital), January - June 1999. Outcome measures: Rate staff adherence protocol; patient characteristics associated poor adherence; demographic prognostic features both groups presentation; duration intravenous therapy, time defervescence, length stay; inpatient mortality rates; drug cost savings per according Results: Overall rate 60%. Patients penicillin allergy were significantly less likely receive treatment (P<0.001). At district hospital, generally older taking more regular medications. each had similar factors presentation. Inhospital (P=0.92; 95% CI, -0.08 0.07), fever (P=0.57) stay (P=0.78) not different between empirically those cephalosporins. Treating saved average $77.44 costs. Conclusion: One implementation, our for treating is proving efficacious, although levels could improve.
