Forward backward translations develop scale. Reliability was measured by test-retest, Cronbach's alpha interclass correlation coefficient (ICC). Face, content construct approved importance score index, ratio (CVR), index (CVI), matrix factor analysis. The SPSS software used data analysis. RESULTS: The 0.917 (CI 95%; 0.901 - 0.931). average scale-level CVI calculated be 0.74; each item higher than 0.83 equal 0.89. analysis extracted three factors including personal activities disorder (PAD), emotional mood (EMD) social (SAD), with more 55.8% total variances.
