摘要: Postarrest myocardial dysfunction includes the development of low cardiac output or ventricular systolic diastolic after arrest. Impaired left function is reported in nearly two-thirds patients resuscitated Hypotension and shock requiring vasopressor support are similarly common Whereas consistently associated with an adverse outcome arrest, association between outcomes less clear. Myocardial arrest develop as result preexisting pathology multiple superimposed insults from resuscitation. The pathophysiology involves cardiovascular ischemia/reperfusion injury toxicity excessive levels inflammatory cytokine activation catecholamines, among other contributing factors. Similar mechanisms occur cardiopulmonary bypass, sepsis, stress-induced cardiomyopathy. Hemodynamic stabilization resuscitation restoration preload, vasopressors to arterial pressure, inotropic if needed reverse effects improve systemic perfusion. Further research define role postarrest on identify therapeutic strategies.
