The decisions and actions taken at this point in their lives will have an effect them years come. This article describes nature of connection between goals outcomes transitions young people, raises challenges effective planning, suggests strategies enhancing effectiveness transition planning. Transition planning teams often include special general education teachers, guidance counselors, parents, vocational rehabilitation related services professionals—and students themselves. Because self-determination is such essential aspect we first discuss as framework remainder our recommendations.
