Polypharmacy Reduction Strategies

摘要: There is no single definition of polypharmacy. Use 5 or more medications commonly used. An alternative, quantitative definition, such as use than clinically indicated unnecessary harmful prescribing, has been proposed. Protocols algorithms to improve polypharmacy and prescribing in older adults have developed. The American Geriatrics Society (AGS) Beers Criteria Screening Tool Older People’s Prescriptions (STOPP) explicit criteria reflect elements that are common across protocols algorithms. Concepts AGS STOPP can be incorporated into reduction strategies outcomes care for adults.
