摘要: Background: The clinicopathologic profile and prognostic factors of osteosarcomas after the age 50 years have been obscure. Methods: Clinicopathologic features were analyzed in 645 patients with osteosarcoma who registered at Tohoku Musculoskeletal Tumor Society National Cancer Center Tokyo between 1972 2002. Results: Sixty-four (9.9%; 34 men 30 women) more than old. most common location was distal femur (n = 13), followed by pelvis 10), proximal 9), fibula 6). Seven (11%) had lung metastasis initial presentation. On radiographs, an osteolytic appearance without periosteal reactions a characteristic feature. Forty-eight tumors (75%) classified as conventional osteosarcomas. Fourteen cases (22%) secondary; postradiation these patients, but there no Paget’s sarcoma. At presentation, misdiagnoses biopsy made 15 (23%) 64 cases. Preoperative chemotherapy given to 22 effect poor 18 (82%). Fifty-four underwent surgery, whereas other 10 treated surgery because systemic or inoperable local conditions. overall survival rate 5 55.5%. Multivariate analysis showed pulmonary metastasis, axial tumor location, larger significant factors. Conclusions: In northern Japan, primary osteosarcoma. Careful radiological examination are mandatory for correct diagnosis. Current is not effective this group. Alternative treatment strategies should be considered.
