摘要: Objective. —To describe the natural history of initially untreated early-stage prostate cancer. A key secondary objective was to calculate long-term survival rates by stage, grade, and age at diagnosis. Design. —Prospective cohort study. Setting. —Population-based in 1 county Sweden, without screening for Patients. —A group 642 patients with cancer any consecutively diagnosed between 1977 1984 a mean 72 years complete follow-up 1994. Main Outcome Measures. —Proportion who died from cancer, 15-year (with 95% confidence interval [CI]), corrected causes death other than Results. —In entire cohort, accounted 201 (37%) all 541 deaths. Among 300 diagnosis localized disease (T0-T2), 33 (11%) In this group, rate similar 223 deferred treatment (81%; CI, 72%-89%) 77 received initial 67%-95%). The 57% (95% 45%-68%) 183 locally advanced (T3-T4) 6% 0%-12%) those 159 had distant metastases time Conclusion. —Patients have favorable outlook following watchful waiting, number deaths potentially avoidable radical is limited. Without reliable prognostic indicators, an aggressive approach early would entail substantial overtreatment. contrast, or metastatic need trials therapy improve their poor prognosis.
