摘要: Objective To evaluate the frequency of bladder outlet obstruction (BOO) and detrusor overactivity (DO) in patients with castration-resistant prostate cancer (CRPC) lower urinary tract symptoms (LUTS). Patients Methods Our prospective urodynamics database was queried. Inclusion criteria were CRPC an International Prostate Symptom Score (IPSS) ≥ 20. Exclusion previous local therapy to gland, known urethral stricture disease, a neurological component LUTS. Twenty-one identified. Urodynamic findings analysed compared those matched cohort 42 benign prostatic enlargement (BPE). Results The median age group 74 years, prostate-specific antigen (PSA) level at time urodynamic study 90 ng/mL. According BOO index, three (14%) obstructed, equivocally obstructed 15 unobstructed. DO seen 12 (57%). Compared BPE group, had cystometric capacities (P = 0.003), less likely have (14 vs 43%, P 0.009) more DO (57 29%, 0.028). Conclusions This generates hypothesis that only minority LUTS BOO, than half DO. LUTS may therefore be seldom attributable but are, least part, related reduced capacity. A investigation necessary before palliative transurethral resection select appropriate candidates. Larger studies are needed confirm our findings.
