摘要: Abstract Backround Sleep disorders are prevalent in patients with advanced cancer. Their impact on clinical outcomes is not well understood. Methods A post-hoc analysis was conducted 361 chemo-naive metastatic colorectal cancer completing twice the EORTC QLQ-C30 questionnaire within a randomized international phase III trial. The study assessed effect overall survival (OS) of subjective sleep complaint, used as normal or time-dependent covariate (TDC), using multivariate Cox proportional hazard model. Prognostic whole population and separately each treatment arm (conventional FOLFOX2, chronomodulated chronoFLO4). Results problems were reported by 202 (56%) at baseline 188 (52%) treatment. independently associated higher risk earlier death (HR: 1.36; p = 0.011), progression 1.43; p = 0.002) poor response (RR: 0.58; p = 0.016). TDC confirmed independent prognostic OS 1.37; p = 0.008), while this only observed univariate analysis. negative value baseline, treatment, greatest chronoFLO4 compared to FOLFOX2. Conclusions Subjective affect chronotherapy effectiveness. There need for well-tuned circadian timing system order increase activity. Prospective studies needed determining therapeutic approaches upon quality life patients.
