Clivus and apicopetroclivus meningiomas--report of 24 cases.

摘要: The authors describe the clinical and radiological features, surgical procedures, outcome of 18 cases clivus meningiomas six apicopetroclivus meningiomas. There were 17 females seven males, ranging in age from 26 to 69 years, with a mean 47 years. All except one showed cranial nerve deficits. Computed tomography accurately demonstrated tumor size location. Cerebral angiography characteristic displacement basilar artery its tributaries, blood supply largely via meningohypophyseal trunk. Magnetic resonance imaging was excellent delineating relationship brainstem major vasculature. approaches used transpetrosal-transtentorial (n = 9), combined transpetrosal-transtentorial-transzygomatic preauricular transpetrosal suboccipital-subtemporal 2), suboccipital 1), frontotemporal (first stage operation) followed by (second suboccipital-transpetrosal orbitozygomatic-infratemporal 1). Seventeen tumors totally removed subtotally. operative mortality 8% (two patients). Twenty patients developed new deficits within 1 month surgery. average follow-up period 5 good or 12 cases. Transpetrosal proved relatively safe effective for radical removal and, two cases, permitted preservation useful hearing. Early diagnosis, use microsurgical techniques, preoperative determination anatomical surrounding structures contributed favorable results.
