摘要: Community Support Connections (CSC) is the result of merger four private not-for-profit community health care agencies. The purpose this research project was to evaluate progress CSC toward its stated goal: improvement coordination and accessibility services. As writing, more than one year into merger, managers, staff volunteers believe that goals serving clients completely through case management are being achieved.
