Decreasing Rate and Extent of Lymph Node Staging in Patients Undergoing Radical Prostatectomy May Undermine the Rate of Diagnosis of Lymph Node Metastases in Prostate Cancer

摘要: Abstract Background At radical prostatectomy (RP), pelvic lymph node dissection (PLND) represents the most accurate staging procedure for presence of (LN) metastases. Objective We evaluated rate PLND use and its count (LNC) over last two decades. also tested relationship between LNC pN1 stage. Design, setting, participants Between 1988 2006, 130 080 RPs were recorded in 17 Surveillance Epidemiology End Results registries. Measurements The statistical significance temporal trends was with chi-square trend test. Separate univariable multivariable regression analyses predictors end points: (1) lack LN (pNx) (2) metastases (pN1). limitations Stage pNx 25.9% patients, higher more contemporary years (30.1% 2000–2006 vs 20.8% 1988–1993; p Conclusions An increasingly larger proportion prostate cancer patients remain without at RP. Fewer LNs removed time, resulting fewer diagnosed stage impact this phenomenon on control outcomes is still to be verified.
